FIRE’s U-4 Regulatory Action Disclosure Solution – Available Now

On Thursday,  May 14, the SEC approved FINRA’s requested changes to the Regulatory Action and Complaint Disclosure Questions on form U4.  These changes went into effect on May 18. In order to help firms collect and file the required responses from each registered rep, FIRE has created an automated solution built on its BlazeQNA technology to deliver the updated questions and report responses.

FIRE’s precision solution consists of a fully customizable online questionnaire, which includes FINRA's Disclosure Questions. All responses are tracked, and any affirmative answers are red flagged and communicated to the program administrator. Periodic reminder emails are scheduled to ensure compliance within the 180 day window, and program administrators can run real-time reports to determine the status and responses from all reps. As part of our service, FIRE will file all “non-red-flagged” responses to Web CRD using the free batch upload FINRA is providing.

BlazeQNA automates the entire process by:

  • Emailing each of your Registered Representatives (RR) a unique username and password to access the questionnaire
  • Requiring each RR to response to all questions and attest that he or she has done so honestly and to the best of his or her knowledge
  • Sending periodic email reminders to those RRs who have not responded
  • Providing you with 24/7 on-demand reports of all RR responses, and an “exception” report identifying those who have not responded
  • Clearly red-flagging RRs for follow-up where responses include at least one affirmative answer
